DIRECT SUPPORT PROFESSIONAL JOB DESCRIPTION
PURPOSE OF POSITION
Developing, training & maintaining the physical, mental and emotional well being of each member in the program.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Demonstrate the ability to relate to and work with adults with disabilities.
• The DSP position requires one to be self-motivated, directing outcomes for all members.
• Provide positive instruction and supervision to members in developing daily living skills and appropriate adult social and behavior patterns by creating the best conditions for the members’ general health & emotional well-being. Be a positive role model for members in behavior and dress as well as exhibiting honesty and dependability.
• Supervise and assist, as necessary, with domestic skills, communication impairments, intellectual impairments, community orientation, transportation, social skills, financial management, health care, behavioral needs, nutritional needs, all manner of personal hygiene cares, and health and safety, as assigned by the Residential Manager.
• Supervise members’ personal allowance and checkbook. Give guidance in the spending of money appropriately and to record expenditures accurately. (Financial)
• Supervise members care and use of clothing and personal property. Assist with such purchases and instruct proper grooming skills and appearance. This includes bathing and application of medications as well. (Hygiene)
• Dispense medications to members and properly record according to medication administration policies. (Medical)
• Obtain and/or maintain a Chauffeur-Class D license and be insurable by our company. You will also be responsible for transporting members to a variety of functions. (transportation)
• Perform all other duties as assigned.
• Daily assess and accurately document member behaviors and compliance as they relate to objectives of the members’ IPP. Respect member differences and personal choice. Have the ability to provide input into members’ IPP strengths, needs, objectives and behavior modification programs.
• Promptly document all incidents, accidents, and behavior patterns on E-docs.
• Attend members’ annual meeting when assigned and accompany members to routine appointments.
• Attend staff meetings and stay informed of all programming and administrative information by the method chosen by your Residential Manager. Promote and practice team involvement at all levels.
• Complete monthly trainings as assigned.
• Review safety procedures with members and document procedure covered and persons in attendance.
• Practice Universal Precautions
• Maintain housekeeping standards and report any items that need to be repaired/replaced to your Residential Manager.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
• Willing to become an Iowa Certified Medication Aide or Medication Manager.

EMPLOYMENT CONSULTANT JOB DESCRIPTION
PURPOSE OF POSITION
Works under the direction of the Director of Job Development Director. The Employment Consultant’s main duties are to assist the member in reaching their employment goal by helping them train, maintain and retain employment, to work with area employers to develop customized employment opportunities for members, and to document and report on these activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Member Employment Consultant Support
1. Coordinate with Director of Job Development and Advancement/Job developer/Placement Specialist, and other Employment Consultants to monitor behavioral concerns when appropriate
2. Identify and communicate member’s strengths, interests and support needs to all involved
3. Identify critical natural supports within the work environment
4. Develop and follow Employment Consultant training plan and develop a corrective action plan when necessary
5. Document and report member information on E-Docs.
6. When placed, assist member with scheduling transportation, inform team, complete placement data, and Employment Consultant plan
7. Provide job coaching, via prompting, redirection, and instruction, to member at the work place
8. Prepare written task analyses, job instructions and/or diagrams for supported employment prompts
9. “Cross train” by learning other Employment Consultants’ training tasks
Maintain Consumer Files
1. Follow Employment Consultant plan and update/change when appropriate
2. Document every activity/ contact with member
Crisis Intervention:
1. Utilize resources available to address member/employer need situations as they arise
2. Notify Director of Job Development and Advancement whenever situations of personal injury, medical emergency, customer dissatisfaction (on any front) or changes in placement status occur
3. Collaborate on a corrective action plan with Director of Job Development and Advancement when necessary
Evaluations of Programs:
1. Evaluate the quality of integration at the job site
2. Evaluate safety issues before member’s entrance to job site
3. Complete Employment Consultant training program on College of Direct Support
Working Conditions:
1. Is not substantially exposed to adverse environmental conditions
2. Must be able to work inside or outside while coaching members

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
• High School diploma or GED required
• Experience in human services field preferred
• Valid Iowa driver’s license, chauffeur’s license, and adequate driving record and maximum vehicle liability insurance
• Ability to communicate effectively in writing and verbally
• Ability to effectively impart instructions to members
• Ability to conform to different job duties, member support needs, and crisis intervention
