Horizons Unlimited has been helping individuals become the best they can be for more than 50 YEARS! At Horizons, we focus on the ABILITIES in people who have disabilities.
Horizons Unlimited is open to anyone who is 18 years of age or older with a documented disability. The current population being served by Horizons includes people with intellectual disabilities, chronic mental illness, and developmental disabilities.

Horizons Unlimited offers residential, vocational and community life engagement programs. Horizons has received the highest accreditation CARF (the Commission on Accreditation of Rehabilitiation Facilities) can offer, which is a three-year accreditation. To receive a three-year certificate, Horizons has put itself through a rigorous peer review process and has demonstrated to a team of surveyors during an on-site visit that its programs and services are of the highest quality, measurable and accountable. 

21st Group Home received the Governor's Award for Quality in Healthcare in August, 2019. Horizons Unlimited also is a member of the National Alliance of Direct Support Professionals. The NADSP works to elevate the status of direct support professionals by improving practice standards, promoting system reform, and advancing their knowledge, skills, and values through certification, credentialing, training, professional development, and accreditation services.
